Hospice Care
Hospice Care provides care for those whose lives are affected by a terminal illness - patients and their families. Care is provided at the patient’s home, in a participating nursing home, or in the Hubbard Hospice House.
The goal of Hospice is to ease the pain and suffering associated with the illness and focus on comfort and maximize quality of life. Hospice team members visit routinely to give care, to answer questions, to teach new and important skills, and to make each day as full of life as possible. Counselors provide care up to one year following the death of the loved one.
Patients may discontinue hospice services at any time they desire. Re-admission to HospiceCare is also possible.
The Hospice Team
Physician - either your doctor or HospiceCare’s Medical Director oversees your medical care
Nurse - makes regular visits and coordinates your plan of care with other team members; on call 24 hours/7 days a week
Social Worker - listens, counsels, and offers resources and support
Certified Nursing Assistant - provides personal care
Chaplain - available for assistance with spiritual concerns
Volunteer - offers a helping hand and a listening ear for the patient and family
Counselor - comforts during the illness and after the death of loved ones
Members of the HospiceCare Team met weekly to discuss the plan of care of each patient. They work together to provide the highest quality of care possible.
